ABSTRACT

Schistosomiasis of the spinal cord is an uncommon but potentially curable form of schistosomiasis, if diagnosed and managed early. The spinal cord is more frequently affected in Schistosomo mansoni or S. hoemotobium infections. This paper describes the clinical manifestations, diagnosis and management of schistosomiasis of the spinal cord in 5 patients attending Shaab and Ibn Khuldoun Hospitals, Khartoum from 1997 to 2007. There were 4 males and 1 female aged 9-45 years. They presented with symptoms and signs due to cord compression at the lower thoracic and lumbar vertebrae. Imaging studies revealed intramedullary masses compressing the cord. Biopsy showed ova of 5. mansoni with surrounding inflammatory reaction. The cord showed demyelination nearthe ova and an associated inflammatory reaction. Patients responded well to surgical decompression and treatment with praziquantel and oral steroids

ABSTRACT

Cisticercose é uma doença endêmica em vários países em desenvolvimento. Embora seja a doença parasitária mais freqüente do sistema nervoso central, o acometimento medular por cisticercos é raro. Pode ocorrer nas formas intradural extramedular, intramedular isolada, intramedular em associação com intradural extramedular, além da forma vertebral. Relatamos o caso de mulher de 53 anos de idade que se apresentou com dor lombar de início agudo, sem outros sintomas. Ressonância magnética (RM) identificou imagem cística na região do cone medular que, no estudo histopatológico, foi diagnosticada como cisticerco. A paciente recusou tratamento pós-operatório com anti-helmínticos e corticosteróides. Após oito anos de seguimento, a paciente exerce suas atividades quotidianas sem dificuldades, e estudos de RM anuais não mostram sinais de doença residual. Aspectos clínicos, fisiopatológicos, diagnósticos e terapêuticos da cisticercose intramedular são discutidos.

ABSTRACT

Parasitic infestations in neurosurgical practice have provided diagnostic and therapeutic challenges to neurosurgeons for many years. In this study we present 15 cases [11 males and 4 females], their ages ranged from 5-40 years [mean age 23 years]. Seven cases presented with hydatid disease [5 cranial and 2 spinal], six cases with bilharziasis [5 spinal and one cranial] and two cases with amebic brain abscess. Clinical assessment, laboratory and radiological investigations have been carried out for all patients. Combined surgical and medical treatment was used in all of them and histopathological examination was done to confirm the diagnosis. All patients have been followed for 2-7 years and the results are discussed in detail

ABSTRACT

Affection of the spinal cord with schistosome eggs cause many neurologic complications. The resulting mass may give rise to controversy in diagnosis. This can be more or less easily judged by MRI. However, radical surgical excision is not always recommended. Biopsied materials for histopathological studies should be done first before any surgical treatment

ABSTRACT

Studies were conducted on the pathogenicity and the route of migration to the central nervous system by the immature larval stage of G.spinigerum in experimental animals. Five mice and one rat showed signs of nervous tissue damage and CNS involvement. The significant findings were massive haemorrhagic areas and blood clot around large lower nerve trunk and spinal cord invasion by the worm. The possible roles of the immature larval stages of G.spinigerum in human was discussed.
